Eve Ackerman, R.N., F.N.P.
Assistant Director
Eve Ackerman is a graduate of Northern Illinois University with a post master’s degree as a family nurse practitioner. She serves on the curriculum development committee, which develops the requirements for each rotation. She also serves as adjuvant faculty for the University of Illinois College of Nursing at Rockford.
P.L. Bandy, M.D.
Assistant Professor/Assistant Director
Dr. Bandy is a graduate of the University of South Carolina College of Medicine in Columbia, South Carolina, and completed her residency at Lancaster General Hospital. Prior to joining the residency, she was teaching faculty at Carle Foundation Hospital Family Practice Residency.
Dr. Bandy is the chair of the residency evaluation and promotion committee and coordinates the inpatient medicine service. Her professional interests include physical fitness and weight management, wilderness medicine, evidence-based medicine and minor office procedures.
Linda Feng Chang, PharmD, BCPS, CDE
Assistant Professor/Assistant Director
Dr. Chang is a graduate of the University of Illinois College of Pharmacy. She completed a general practice pharmacy residency at the Chicago VA. In addition to her clinical practice in ambulatory care at the residency, Dr. Chang is a clinical assistant professor in the department of pharmacy. She is responsible for journal club, the evidence-based medicine curriculum, clinical therapeutics and elective rotations. Her professional interests include alternative medicine, chronic disease management and nutrition. She is actively involved with the Northern Illinois Diabetes Coalition.
Sherry A Falsetti, PhD
Associate Professor/Assistant Director
Dr. Falsetti is a graduate of the University of Missouri-St. Louis with a PhD in clinical psychology. She completed an internship and postdoctoral fellowship at the Medical University of South Carolina, where she also served as faculty for nine years and served as the Director of Training for the Postdoctoral Fellowship Program and the Director of Clinical Operations at the National Crime Victims Research and Treatment Center. She has won numerous awards for medical student, intern, and residency teaching and has been a National Institute of Mental Health grant recipient. She maintains an active clinical practice and specializes in the treatment of mood and anxiety disorders, the psychological impact of chronic health problems, and the interplay of physical health and behavioral sciences. Dr. Falsetti serves as the chair of our faculty development committee.
Leslie Filer, R.N., L.C.S.W.
Instructor/Assistant Director
Leslie Filer earned her bachelor’s degree in nursing from Northern Illinois University and later her master’s degree in social work from the University of Illinois. She has 23 years experience in the healthcare field, spending over a decade working with individuals in the community setting, addressing healthcare and psychosocial needs of clients and family members. Her special interests include mental health and working with the terminally ill.
Kenton Lee, M.D.
Assistant Professor/Assistant Director
Dr. Lee is a graduate of John A. Burns School of Medicine at the University of Hawaii. He completed his residency at the University of Illinois College of Medicine at Rockford and was chief resident during his third year. Dr. Lee serves as the chair of the curriculum development committee, and is responsible for the pediatric and dermatology rotations. His professional interests include residency education, faculty development and practicing the full spectrum of family medicine.
Richard Londo, M.D.
Assistant Professor/Assistant Director
Dr. Londo is a graduate of the University of Illinois College of Medicine in Chicago. After completing his family practice residency at the University of Illinois College of Medicine at Rockford, he practiced in his hometown of Morrison, Illinois, for over 13 years. Dr. Londo first joined the residency faculty in 1989. He then worked for five years with the Rural Medical Education program at the University of Illinois College of Medicine at Rockford before returning to the residency, where he concentrates on geriatric care, in addition to the full spectrum of family medicine.
Michael Polizzotto, M.D.
Assistant Professor/Clinic Medical Director
Dr. Polizzotto joined the Rockford Family
Medicine Residency program after completing
11 years of active duty with the United
States Navy Medical Corps. He taught
in the family medicine residency program at Naval Hospital Camp Pendleton
in California for four years, where he was twice selected by the
residents as FP Staff Teacher of the Year. Dr. Polizzotto graduated
from the University of Vermont College of Medicine in 1994 and completed
his residency in family medicine at Camp Pendleton in 1999. Dr. Polizzotto
is responsible for the clinical teaching in the Family Health Center,
Inpatient Family Medicine Service and surgery rotations. His professional
interests include residency education, diabetes care and clinical
quality improvement.
Eduardo Scholcoff, M.D.
Associate Professor/Assistant Director
Dr. Scholcoff is a graduate of the University of Buenos Aires, Argentina, School of Medicine and the OB/GYN residency at the University of Buenos Aires. He was an OB/Gyn resident instructor before completing his family practice residency at the University of Illinois at Rockford. After completion of family practice residency, he joined the faculty and has been the operative obstetric track coordinator since 1997 and is the chair of the credentialing committee. Dr. Scholcoff has won numerous teaching awards including the UIC college faculty distinguished teaching award.
Adriana Tobar, M.D.
Assistant Professor/Assistant Director
Dr. Tobar is a graduate of Pontificia Universidad Catolica Del Ecuador. She completed her Family Medicine Residency at the University of Illinois College of Medicine at Rockford. Dr. Tobar was involved in the organization and foundation of the Scientific Society at the medical school at the Pontificia Universidad. She is responsible for the emergency medicine, radiology and cardiology rotations. Her professional interests include care to the underserved, maternal child health and practice-based research.
Farion R. Williams, M.D.
Associate Professor/Program Director
Dr. Williams is a graduate of the University of Texas Health Science Center at San Antonio. He completed his residency at the University of Kansas School of Medicine at Kansas City and was chief resident during his third year. Before joining the residency as the program director, he was the associate program director for Baylor College of Medicine’s Residency Program in Family Medicine. He has also worked in a community practice in Dickinson, Texas. In June 2000, he completed the University of North Carolina’s fellowship program in faculty development. His professional interests include residency education, adolescent medicine and public health issues.
